The following forms MUST be filled out and handed in to your team manager:

• ** Player / Parent Manual (Last page must be completed and signed)

• ** Parental Pledge & Code of Ethics

• ** USA Hockey Consent to Treat/Medical History Form

• ** USA Hockey Participant Code of Conduct

**  All relevant pages must be printed, filled out, signed and submitted to your Team Manager before player can participate in any team activities.


All parents MUST also read and understand our TRHC Code of Conduct as well as USA Hockey's Zero Tolerance Policy.
